The paper by Wu M., Wang D.L., and Brown G.J. (2003): A multipitch tracking algorithm for noisy speech. IEEE Transactions on Speech and Audio Processing, vol. 11, pp. 229-241, is a good starting point. On their page, you will also find updated work and computer code.
